Before your first session, please complete and sign the New Client Demographic Forms.
You can easily complete these forms online. Click the link below to create an account and submit your information through our secure Client Portal. Once your account is set up, we will contact you to confirm your insurance and schedule your first session.
If you are unable to complete the forms through the Therapy Appointment Client Portal and need a printable version, please contact us to request the New Client Packet and submission instructions.
